Please click below to complete the 4th grade application to SMART in English. The final deadline is April 29, 2022, and we must receive all parts of the application in order for your application to be considered complete.

 

Before completing the application, please gather the following required documents. You will be asked to upload these documents before submitting:

• 2021 Tax Returns. If you have not yet filed taxes for 2021, please submit your 2020 Tax Returns. 

• Your student’s 3rd and 4th grade report cards.If your student is an SFUSD student, you may download these from Parent Vue.

 

The following steps are required for your application to be considered complete:

• The student is a current 4th grader (during the 2021-22 school year)

• The student will be a first-generation college graduate. This means that the student will be in the first generation of their family to graduate college (they may have siblings that are college graduates or in college, but neither parent is a college graduate)

• Your family meets SMART’s income qualifications.

• Your student must attend the mandatory in-person summer program from mid-June through July.

Antes de completar la solicitud, reúna los siguientes documentos requeridos. Se le pedirá que cargue estos documentos antes de enviar:

• Declaraciones de impuestos 2021. Si aún no ha presentado los impuestos para 2021, envíe sus declaraciones de impuestos de 2020.

• El estudiante está actualmente cursando el 4to grado (durante el año escolar 2020-2021)

• El estudiante será un egresado universitario de primera generación. Esto significa que el estudiante formará parte de la primera generación de su familia en graduarse de la universidad (el estudiante puede tener hermanos que sean egresados universitarios o estén en la universidad, pero ninguno de los padres puede ser un egresado universitario)

• Su familia cumple con los requisitos de ingresos de SMART.

• Su estudiante deberá asistir al programa obligatorio de verano, en persona, que empezará a mediados de junio y terminará en julio.
